Registration Procedure

>  Registration must be done by school principal or teacher
>  To  get an accurate school report, a minimum of 10 participants per subject are required

 

Registration procedure for ICAS Entry are as follow:

A.    ENTRY FORM

Download Entry Form (.Pdf)

  • Use only black or blue pen or pencil. Do not use a felt or red pen
  • Print neatly and clearly. Use BLOCK CAPITAL LETTERS ONLY
  • Do not bend, fold, staple or place stray marks on the Entry Form
  • Important: Fill in the ‘School Name’ column with the official school name as registered at Depdiknas. The school name will be printed on the school certificate and the student certificate

B.    TEST CANDIDATE LIST (TCL)

Download Test Candidates List

  • TCL is a list of student’s name who will participate in ICAS
  • Print the test candidate’s name on the letter head of your school
  • Print neatly and clearly. Use BLOCK CAPITAL LETTERS ONLY
  • Print the test candidate’s name separately based on their ICAS subject entry and year level of education

C.     PAYMENT
Total payment should be transferred to:
